Hey MT, you know much about langerhans cell histiocytosis? My wife just got diagnosed last week from a skin biospy they did a couple weeks ago. Met with the hematologist yesterday. At this point they know it’s in her Pituary gland and skin but they’re running a boat load of tests to see what other organs are impacted.
Extremely extremely rare the doc said. Said only like a few hundred people in the world have the adult version of it.
Doc said it’s not curable only manageble, but generally it’s not life threating, although she said she couldn’t give us any positive news until they ruled out all the organs its impacted. Hopefully it’s not anywhere else.
I know it quite well. Obviously radiology plays a large role in diagnosis and ongoing evaluation of treatment, and within the field of radiology neuroradiology (my subspecialty) plays the largest role. It is of course a disease that is in the differential of a lot of findings, especially in kiddos, but about 1/3 of cases pop up in adults, usually in their 30s – it is usually multisystemic in adults. It is a rare disease, absolutely.
Agreed the less organs involved the better. The prognosis is much more guarded the more systems involved.
Dang, hopefully she’ll be alright. Got about every type of testing coming up the next couple weeks (PET, CT, MRI, blood, bonemarrow, etc.) Sucks, but hopefully it’s not in those critical organs and they can suppress it somehow

OK fellow medical professionals, got a question for you. Recently I’ve had a couple people admitted for rehab following hip fractures. They both slipped/tripped, fell, and fractured their hip.
But, because they both have a pre-existing condition of Osteoporosis, their fracture is classified as Pathological rather than Traumatic.
Anyone aware of why this is occurring?
I would say it is because the expanded ICD-10 coding requires precise categorization of such fractures…for example: M80.01 Age-related osteoporosis with current pathological fracture, shoulder
If the coder does not tease out that detail from the clinical notes then there is a risk the claim gets denied.
I doubt there is any significant difference in reimbursement. But that code would then link to outcome measures like treatment of osteoporosis, ongoing DXA scans, etc…part of the improvement of population based outcome measures.

You’re likely correct that there will be no change in reimbursement, but seems like they’re trying to pick nits. People fall and fracture hips all the time and they don’t have Osteoporosis.
We were taught that a break is deemed a pathologic fracture when force or impact didn’t cause the break to happen. Instead, an underlying disease leaves bones weak and brittle. You may move wrong or shift your body weight in a way that puts pressure on weak bones and they snap.
The one lady is almost 300#. Sure, she has Osteo, but 300# coming down on the greater trochanter is gonna cause a Traumatic Fx.
I don’t do anything with fractures (that’s trauma) unless Ortho manages to get someone’s ex-fix infected even though got the “magical” Ancef.

We’ve been streaming a Spanish series on NF called “Elite” based on the goings on at an exclusive Spanish private school. If their depiction of all the guy on guy fucking going on despite the abundance of hot Spanish girls in skirts is accurate I would say the population of Spain will rapidly decline. Vikings Get: 2023 third-round pick (No. 91 overall)
Bills Get: RB Dalvin Cook
The Vikings have already parted with standout receiver Adam Thielen this offseason, and it wouldn’t be a total shock if running back Dalvin Cook is next.
Cook is slated to carry a cap hit of $14.1 million in 2023. While $2 million of that has already been guaranteed, Minnesota could still save $7.9 million by trading the 27-year-old. Before Thielen was released, there was some buzz that the Vikings could do just that.
“Multiple teams believe Minnesota has had trade talks centered around running back Dalvin Cook,” ESPN’s Jeremy Fowler wrote last month.
From a financial standpoint, it could make a lot of sense to move Cook and dip into this year’s running back class. The Vikings just signed backup Alexander Mattison to a two-year extension, and they’re high on 2022 fifth-round pick Ty Chandler as well.
Minnesota might be able to find a taker in Buffalo, which could pair Cook with his brother and fellow running back James Cook. Buffalo also added Damien Harris to the backfield this offseason, but Harris doesn’t offer the same dual-threat ability that Dalvin Cook can.

What do these words mean?
You know when you listen to the radio or a podcast and it sounds like the voice is right up front and sounds real close? It’s because they are running the microphone through a vocal preamp with outboard compression. DBX 286 is pretty much an industry standard. And it has things on there like de-esser that takes that hard s noises away and such.
That sounds like some sort of witchcraft to me.
I’m telling you, when you’ve been in some high end studios and record yourself it changes your perspective in music when you can tell how they are cheating lol. Which I mean, a vocal pre-amp isn’t cheating. It just makes shit sound good. If you want to sound professional on your Podcast get a DBX 286 or something similar.

Did you see the interview where the guy who did that What is a Woman documentary was talking to a trans woman who was an EMT. All he was trying to do is get her (and I’ll call her a her, who gives a shit?) to admit men can’t have babies. So she tried saying well someone biological females can’t have babies too (which got shot down with logic as well) but then that guy says, OK, so you are an EMT. If you got called to an emergency on a biological male, would you check to see if they were pregnant before administering any kind of help? I am saying even if you think it’s a mental disorder which I agree with, what’s the answer to treating it? It’s not like bi-polar where you can get on things to right your hormones and such. I think many of the cases aren’t necessarily a chemical imbalance as much as a reaction to the world they live in. That could be from a trauma, from feeling mistreated or ignored and seeking attention, being raised in an environment that doesn’t make them feel worthwhile, etc. Counseling would help and just let kids become adults and grow up before trying to pump them with any drugs or hormones.
Personally I think a big chunk of it comes from not being accepted for who you want to be or who you want to love. Think about it. If you were gay, wouldn’t it be much easier if you could just be a woman. Then you could love women and not get side eyed and judged from everyone. I think we just need to accept people for who they are. Let people explore their sexuality and gender and I think more often they will end up not going under the knife or doing hormone therapy.

We are going to go straight from winter to summer here. Around 20 this morning, around 80 next week.

I think we roster build in this draft instead of going QB hunting. Too much talent and we have a boatload of FA next year that it would be nice to replace some of those guys this year. We can pick a lot of players with trade backs and there will be a lot of opportunity to do so day 2 and 3 as teams want to move around and get their guys. This mock shows the potential of who we could bring in. I had to post two picks of this mock to fit them all in.
