I’m sitting here watching the Saints-Jaguars game. The Saints just came back to tie it at 24, but it’s late and I highly doubt I’ll watch this one to the end. My opponent in the fantasy league has both Etienne and Kamara, so I’m getting smoked so far this week.
I hope everyone’s work week is going well. I really love fall. Football is in full swing, it’s bird season, the walleye rush is about to start, and the weather in North Dakota this year is surprisingly tolerable. Heck, it was 70 here yesterday. We don’t get many days like that up here in late October, I’ll take it. I can recall many years being buried in snow by this time of year.

Judd Zulgad says that the Vikings’ reliance on Alexander Mattison in the run game raises questions. I’m a bit perplexed myself. Akers appears to be an upgrade over Mattison, especially in the feature back role. He certainly appears to have more home-run power, and he would most certainly boost the Vikings current rushing production. I assume the coaching staff have their reasons for rolling with Mattison, but to we mortal fans, it seems counter-intuitive.

When I was eating lunch I turned on Rutgers and whoever they were playing (Indiana?). The whole 20 minutes I was eating, there was about 2 passes. Rutgers ran the ball over and over and over. I thought I had time traveled to 1901 or something.

Just learned that you can put Dry Ice in beverages for the spooky Halloween effect (fog, bubbles).
Evidently the ice sinks to the bottom and so you can drink the beverage safely just be sure not to ingest the Dry Ice itself.
Not something I would want to try with kids drinks or with drinks with too high an alcohol content. Maybe a punchbowl would be ok??
I watched an episode of some bartender reality show, and one person used dry ice in their cocktail, and the judges refused to drink it because it was too dangerous.
I was surprise to learn that it can be put into drinks.
With obvious warnings and caveats included.
Never around kids and drunks I don’t think.
I guess my point is, if professional bartenders wouldn’t drink a drink with dry ice in it, it must not be safe to do.
I don’t think I would try it.
Maybe a bunch bowl if it is sitting at the bottom and you scoop just from the top??

I can babble about golf. I golfed 9 today, hit the ball well, did not keep score, lost two balls in the water. Came home and took a nap while watching football.

I have heard a couple of posts saying he was missed.
About the same as I have heard say ‘not so much’.
Lifer used to be a guy that I couldn’t stand. Now, he doesn’t bother me that much. Maybe I have grown as a person (unlikely), or maybe it is just exposure therapy. There is however another guy that comments right now that is a burr under my saddle with just about everything he says. Sometime there are people that just don’t mesh, while that same person doesn’t bother other people.
Depends on which version we get.
The calm/thoughtful/articulate one isn’t bad.
But that one doesn’t last very long.
I have often been concerned about his mental health.
He is very unstable.
Luckily he never worked at a post office.

It was a fun week in my bookstore. I added an eclectic group of books. From an 1857 religious text to a 2002 limited edition of horror/fantasy/scifi short stories that was signed by all 19 contributors to a signed edition of a children’s book by an author that lives here in Wisconsin. Of course I also had some generic stuff, but the other stuff is what makes it fun.

Got my Nephew a 100+ year old hymnal at a book fair. It was in German but he wanted it for the music as he was learning to play the guitar.
Surprising it was only about $5. Nice when you can support the young ones on the cheap.
Most of those older hymnals do not have much of a dollar value. They are interesting to look through however.

I read Myra Breckenridge as a youth.
I am still trying to recover.
Sadly the book didn’t have Rachel Welch in it like the movie did.
This book is supposedly a non-fiction narrative from 4 different prostitutes. I just glanced at it and parts looked interesting, but when I read part of the introduction, I realized that the author was just a crazy feminist.
I LOVE bookstores. Unfortunately many have closed due to the on line thing. We recently took the grands to an awesome one in Lexington, where I took my daughter often when we were living there. Not only did it survive, it actually expanded. Not really any decent ones left in Louisville.

I read it on this site earlier in the week 1 in rushing and 3 for passing. I can’t find it anywhere else though..
In Rushing Yards Cleveland is 9th and we are 27th.
In O-line ranking, that is subjective/opinion, PFF and NFL think we are better than most fans here do.
Stop trying to give me hope…If we are 1 in rushing Oline why did we gain 44 yeards on 20 first downs vs the Bears.

I asked a inmate once how much time he had left after he referenced only having ‘a minute left’.
He said 13 years.
I said ’13 years = a minute’?????.

He said ‘less time than my dad has’.

But once you have said mindset it can be entertaining.
How long was his sentence? If he’d already served 30+ years 13 probably does seem a minute to him.
Don’t know. Could have looked it up if I cared enough at the time. He looked to be in his 30s though.
We were not computerized in our Housing Unit at the time and going to ‘records’ wasn’t worth the effort.
Still gave me an insight to how they think.

I always found that for me Counting Down and focusing on how much time I had left made it more difficult for me.
You had to remember to get things done related to future moves but daily counting? Didn’t work for me.
I have a formula in my retire spreadsheet that calculates the days. I don’t look at it all the time, but on occasion I like to look at it and see my progress. I have a good job and a good side job, but I am ready for the side job to be my only job.

Not like me, retired almost 4yrs now and still trying to figure out what to be when I grow up.

Good for her. My wife is a teacher also. She dislikes doing anything with our finances and she trusts me to handle it all. She keeps telling people that she will retire when I let her. She has a great sense of humor.
The Teacher had her retirement date set in her head when I met her.
She is super organized and proactive.

I tell her that is why we make such a good team.

The Teacher went to the ER a few weeks ago, they figured she had an Abscess and had/was Sepsis. So they drained the abscess, admitted her so they could get anti-biodics into her via IV and monitor her to insure the abscess stayed drained. She stayed two nights until they cleared her.
This week the insurance (Blue Cross/Blue Shield) denied the claim saying it was just ‘wound care’. Working on getting the hospital/health insurance to work it out together.

It is so infuriating, our health care system is a mess of inefficiencies and overpaid Radiologists (O.K. that was a cheap shot).
This is why I believe we will adopt a one payer, government run system.
Now under that system, your wife’s care would have likely been delayed for several hours and she would be dead now, but hey you wouldn’t be worrying about the insurance.
So per the insurance she should have refused the Dr’s orders for admittance because she would know better then them?
One of the symptoms of Sepsis is lightheadedness/change in mental status.
Yup, perfect for refusing care and going home.
Just remember the ER is not in the business of optimizing care and being cost efficient. They are in the business of getting you out of the ER. And…not once do doctors stop and say will this inpatient visit be covered by insurance. It is what it is. I would imagine there will be a medical necessity convo between the insurance doc and the admitting docs/discharge notes and that will be the end of it.
They wanted to wait for an MRI before releasing her.
But that was not related to the original issue, they just saw a ‘smudge’ on her kidney that might have a cyst and wanted to check it out.
We said ‘yeah will do that outpatient rather than wait another day for the MRI to have an opening’.
Oh that’s a monster pet peeve of mine…inpatient billing is captitated by DRG and extra imaging done as part of entering the radiology vortex just gets sucked into the admission cost. F that. You can follow that questionable lesion as an outpatient.
Might have to explain Diagnostic Related Groups to the rest of the class.
Paid based on the condition that landed them in the hospital, not based on the care that is given or how long they stay.
Fixed price by condition.
They were kind of shocked that we refused the MRI.
Kaiser was having it’s strike so more people coming in needing an MRI.
ER patients had a higher priority, with good reason.
MRI was ‘across the street’ so medical transport was required.
MRI broke for a few hours, pushing her further down the queue.
It might have another day (or two) if we waited for the hospitals machine.

Also, we deal with families all the time who bring us another therapy order from the MD after insurance has already cut them off.
We’re the ones stuck telling the family member “The doctor can write all the orders he/she wants, your insurance won’t pay for it.”
We didn’t see the denial until it was mailed to her two weeks after the fact.
Paperwork was dated while she was in the hospital though.
I am betting that for SoCal that abscess was drained by radiology. Maybe not. But yeah hoho haha what you say rings of truth, she wouldn’t have died but the time waiting for that drain (which may have never happened) would have been significant and she may have just been hospitalized on IV antibiotics for a week or so before discharge on PO abics.
yes, I know she probably wouldn’t have died but her care likely delayed. Drs Salaries are not the problem with the US health system. I can think of several real problems though… fragmented care, 26,000 different insurance and billing procedures, way too much litigation, just to name a few.
We counted 5 Drs that visited/talked to her in her 36 hr stay.
Started to think they wanted to be able to get some billable time by stopping by.
Yep, I often swing by labor and delivery to admire the gravid bellies and then the nursery to bounce the little babies. Parents will ask who are you? (oh Dr. MT a radiologist) is there something wrong with our baby! (oh no they haven’t even ordered imaging) why are you here? (just to wet my beak with a small percentage of your bill.)
Would hope that the gougers make enough without that kind of stuff but human nature being what it is…….
Had a shyster lawyer BIL who would/could talk all day about how to make $$ with creative billing.
Helps you see behind the curtain enough that you have to watch out for it.
Heh I hope you know it was sarcasm. Actually had an issue with my MEC involvement where a surgeon was not rounding daily on patients (they were being seen by hospitalist as well). That’s a big no-no both from compliance and billing standpoint, plus not good customer service to say the least.

Interesting statistical fact – do you know that when you are in the hospital, your odds of dying are inversely proportional to the number of physicians you see?
The more physicians you see the more likely you are to die.
Of course the reason is obvious, but I use that example when I am teaching statistics – an observed relationship is not evidence of causation.

The same is true of the number of imaging studies performed as an inpatient, for the same reason.

It’s an unfortunate aspect of our system that when you are at your sickest you must be thinking about getting authorization from insurance. My wife had an echocardiogram ordered recently and first thing I did was call the business office preauth person here at the hospital. Gave her the ICD-10 code and the CPT code (although she could look it up in the EMR), got a call back three minutes later “you’re good”. Just received the EOB, that sucker was $1600 and after the usual deductions “we have billed insurance for $679”.
An interesting aside (I am a long ago former Albertan, that is where my mom and sisters live), Alberta is thinking of pulling out of the Canada Pension Plan (kinda like Social Security) as they are getting bent over by carbon taxation related to the predominant business in the province ie oil and ag. Some people have reported a 10x increase in heating bills related to carbon tax. Now what CPP has to do with carbon taxation and how that will make things better for Albertans I don’t really understand but it is interesting watching the implementation of programs designed to curb carbon emissions and encourage usage of renewable resources.

A fair question. I can only answer from my personal perception as one from the Evangelical side of the fence..
We believe you need to accept the free gift of grace that Jesus Christ purchased on our behalf. This free gift makes us right with God despite our on-going sinfulness. This gift was given to all humanity out of love.
Now, a child cannot understand this concept. But a child does understand love, which is a big part of the equation. So yes, first and foremost we teach our children that God is a loving God.
Later, they are introduced to the concept that they are indeed sinners and in need of God’s forgiveness through Christ.
But then why only tell the good part of the story up front? I agree they can’t understand the consequences side but to just leave that out and hook them with the nicety side… It’s not fair, it’s not right imo.

Interesting point. A close friend and study buddy from residency has a Down’s child. They were low risk and the only finding on prenatal exams was a solitary intracardiac echogenic focus on ultrasound, which was dismissed as of no clinical significance.
Their boy is a great kid, obviously medical advances have greatly expanded his life span and options in life.
My friend and his wife are deeply religious.
So when I had a friend come to me with questions about the solitary IEF I described in my report, I called my old friend up. It was an interesting conversation. He said my advice is she should do all available testing (including amniocentesis or chorionic villus sampling)…it is better to have as much knowledge as possible rather than have it sprung on you as a surprise at birth. Really surprised me.

This just in King Charles has proclaimed that all able bodied men considering enlisting in the Canadian military just tie a cinder block to their ankle and step off a pier. Think of the massive savings in military benefits. The vast majority of the HCW’s in Canada are paid by the government. For that matter ALL of the military is paid by the government. Does that mean the government is responsible for Russell Williams’ actions? I’m with you, Hun. We can all understand the terminally ill in pain, however it is definitely a slippery slope. I saw in Canada a 20 year old boy was assisted in suicide because he was depressed. A minor child was also assisted because she had a disability. I don’t care whether the government officially said, all disabled children should be encouraged to commit suicide. The fact that it happens in situations like this demonstrates that life isn’t valued like it used to be.

The East Germans were actually letting Berliners cross at Checkpoint Charlie. At Brandenburg there was a heavy police/military presence keeping the East Berliners away from the wall. Then word must have been spread that people were crossing at Charlie, and they broke through the police/military, we climbed up on the wall and started pulling them up and over.

Well this thread devolved in a predictable fashion, but I’ll say three things:
Treated over 150 pts with suboxone, opioid use disorder, heard all the horror stories. I think “my” long term “success” rate was about 10%. Often I found myself thinking that drug dealers should get capitol punishment. Not sure I believe that, but thought crossed my mind.
It will be interesting to me to watch how long it takes for Canada to move from “allowing assisted suicide” to encouraging it. Perhaps they already have.
Mrs. Jones this bone cancer is going to get really painful near the end, how about we just give you this pill and you won’t have to experience it (and as a bonus we will end up saving our government run health care system some money).

The health care systems is another debate. FTMP Canadians really like their health care system, and a lot of Americans complain about theirs…but I see the gap between medical practice here in the US and medical practice in Canada as a gap that could not possibly be bridged.

Yes, I am aware of the treatment gap. I do believe the US will move to a Canadian style system in the not too distant future.
I tell my liberal friends be careful what you ask for, universal government health care sounds great but it comes with some real limitations you will not like.
I honestly do not see it. It took 25 YEARS of lobbying and the community raising the funds themselves to purchase a CT scanner in Wainwright, Alberta, population 6200. The next closest CT scanner is 60 miles away. By that metric, vast swaths of Montana and North Dakota would not have cross sectional imaging. Think of the lawsuits and news reports of Granny Smith waiting six months to find out she had cancer, etc…
Any time you allow a MASSIVE bureaucracy to run something, expect long waits and if something goes wrong, there will be no one that can make a decision. And once a “decision” is made, expect that it will be run roughshod over anyone that has an exception. Expect no compassion or understanding, just sterile rules.

I watched the first one maybe a year ago. It did not inspire me to watch the rest of them.
We made my son, who was fairly young at the time, read each book before we went to the movie…It was a great way to encourage him to read.
And now he is a veracious reader.
As a book store owner I thought you would appreciate that.

I always hear people say, “the book is so much better than the movie”, but they’re all just full of shit, movies are way better and everyone knows it.
What if it’s in another language so you have to read the subtitles the whole movie?

Yeah. One recentish example in my opinion is Lone Survivor. It wasn’t a bad movie if you hadn’t read the book. If you read the book, the character Marky Mark plays is a pansy compared to the real guy.
My example for this is the Stephen King movies from the 70s and 80s. Many of them were tv level quality (really bad). The books are much better.
If I see the movie first, I like the movie. But, if I read his book first, then watch the movie, the movie sucks. Best example of this is The Dark Tower.

While I think the LOTR movies were good, there is no way they compare to the books.
In fact, I have not seen one movie that I think is better than the book it is based on(in cases where I have read the book, obv)..
That said, watching a movie for a few hours is certainly easier than reading.
