What an exciting evening I had last night! My wife called me in the middle of the afternoon to tell me that she’d gotten two tickets to a banquet for Cross Training, a local ministry, and Dalvin Cook was the speaker. So, kiddo and I attended the event. Dalvin showed up a bit late, as the coaches called a last-minute practice for running backs only, but he did make it.
He shared some amazing insight about his faith, how he grew up, how football saved him, and about his journey to the NFL that led him to Minnesota. I’ll tell ya’, my respect for this man grew a LOT last night. What a great human being. Humble, reverant, and honest are the words that come to mind first.I took a video of the event, but it’s too large to share. I’ll have to play around (and talk to someone who knows what they’re doing), and try to share it with you here.

Just did a bone biopsy. I forgot how hard it can be to get through thick dense sclerotic bone. When I was gently transferring the samples into formalin etc I was thinking why are my hands shaking so bad? Just because I have an audience of technologists, nurses and CRNA? When I got back to my office I realized the problem, even typing this is hard, my stupid arms are quivering from the effort.
How do they localize for the procedure and pain…does an anesthesiologist do a block?
No, nothing so dramatic. Interestingly, here at the small hospital I’m based at this is the first time the anesthesia team (nurse anesthetists) have been involved in a procedure so I was nice and deferential to them. I just couldn’t see doing a bone biopsy (ow) without their help. There is no radiology nurse here to assist (moderate sedation was performed by a radiology nurse tasked to monitor the patient with no additional clinical responsibilities, under my supervision). No way I’m taking on the task of monitoring sedation while performing a complex procedural task, that’s just asking for disaster.
CRNA and two nurses did the intake on the outpatient, transferred to CT, I met and consented patient, they started giving sedation with midazolam and fentanyl IV (already all hooked up to monitors and oxygen)…still pretty active so titrated with propofol (non anesthesia providers cannot obtain propofol from hospital formulary for patient safety reasons). By the time I had the leg prepped and draped patient was sleeping. I localized the spot for access using CT, numbed the skin with lidocaine, flooded the cortex of the bone with bupivacaine, and started driving an 11 gauge Jamshidi needle into the cortex. Slept throughout thanks to the “magic milk”.
Michael Jackson enjoyed the magic milk…a little too much unfortunately.

Sometimes shit is complicated and expensive. Either you throw your hands up and say its too hard, or you find the solution.
I’d much rather my tax dollars went to something that will actually benefit society, vs a lot of the bloated BS they spend trillions on.
A shift toward electric cars would need to be done a similar way to R&D for new prescription drugs, IMO. Give the free market sufficient incentive, and let some go-getters figure it out and get rich.
As long as they aren’t gouging, go get it.
All this is pretty far in the future, but the longer you put it off the more complicated/expensive it is going to get.
